**This ordering system is for use by Immunization Service Providers, School Health and Child Care personnel in San Diego County Only.
We cannot supply materials to any individual or organization outside of San Diego County. Thank you.
**NOTE: Only 1 order per agency per month will be processed.
Make your selections from the sections listed below. Enter the quantity for each item in the appropriate box. Most items are ordered individually but stickers are ordered by the roll, packaged items are ordered by package, and tablets are ordered by tablet.
Attention School and Childcare providers: To order the Personal
Belief Exemption Forms and the Blue Card Stickers for the Pertussis
Booster Immunization (Tdap) Requirement, please contact Jennifer
Sterling, School and Childcare Coordinator, at (619) 692-8366. As stated above, we cannot supply materials to any individual or organization outside of San Diego County.
To order supplies of the School Entry Health Checkup Requirement (Green Form), please visit the CHDP Program Green Form ordering page by clicking here, or call the CHDP Program at (619) 692-8808.
PLEASE NOTE: To help reduce program costs, we are requesting that immunization materials orders be picked up at the Immunization Program Office. Driving directions to our building, the Rosecrans Health Services Complex at 3851 Rosecrans Street in San Diego, an area map and instructions to find our office once you arrive at the building can be viewed and downloaded by clicking here.
Pickup days and hours are:
Tuesdays from 8 a.m. to 12 p.m. and Fridays from 1 to 5 pm, except holidays; or by special appointment.
If you are not able to pick up your order, please call the Immunization Program at (619) 692-8825 and request alternative arrangements for delivery of your materials.
