School and Child Care Annual Immunization Assessments: You Can Submit
Yours Online!
We are
pleased to announce you may submit your annual immunization
report on children enrolled in Child Care/Head Start
facilities electronically this year at www.ShotsForSchool.org.
Please read the packet (arriving soon) to determine
your user name and password. See below for instructions and
more information about different types of school and child
care immunization reporting requirements.
California law requires that all child
care centers, day nurseries, nursery schools, and development
centers submit the ANNUAL IMMUNIZATION REPORT ON CHILDREN
ENROLLED IN CHILD CARE CENTERS (CDPH 8018) every year. Head
Start programs are included in the category of child care
centers. This report includes the immunization status of all
children from ages 2 years through 4 years, 11 months. While
the report does not include children who are younger than 2
years, these children must be checked as often as necessary to
assure that their immunizations are up-to-date.

California law requires
that all public and private schools with kindergarten students
submit an annual immunization assessment (California Health
& Safety Code §120375(c); California Code of Regulations
§6075). You can go to www.ShotsForSchool.org/reportingtools
for help in completing this portion of the Kindergarten

California law requires that all public and private schools
with 7th, 8th, 9th, 10th, 11th and/or 12th grades submit a
pertussis (Tdap) immunization assessment (Health and Safety
Code, Section 120375). Schools must report on the status of
ALL students in 7th – 12th grades enrolled for the 2011-2012
school year.
