Adult Immunization Record for patient charts. It is available free of charge for your practice.  It contains all required information to be charted including date of immunization, lot number, and immunizer’s initials.  Adult Immunization records can be ordered from our website at http://www.sdiz.org/catalog/catalog.asp then select IMM-542A and enter the quantity desired. 

Vaccine Information Statements (VIS) for all vaccines should be given to patients prior to receiving immunizations. They can be printed or downloaded in over 30 languages at: http://www.immunize.org/vis/

Free online interactive training is available for medical assistants in storage & handling, vaccine preparation and administration. Go to www.eziz.org and click the EZ-IZ Training button in the upper left part of the screen. Many additional training aids are available at this site.

Temperature logs - In order to prevent financial losses and ensure the efficacy of vaccines, temperatures must be maintained within specific limits and should be checked twice daily; first thing in the morning and prior to leaving in the evening. To confirm temperatures were checked and maintained within proper limits, temperature logs can be ordered from our website at http://www.sdiz.org/catalog/catalog.asp.
