Adult Immunization Record for patient charts. It is available free of charge for your practice. It contains all required information to be charted including date of immunization, lot number, and immunizer’s initials. Adult Immunization records can be ordered from our website at http://www.sdiz.org/catalog/catalog.asp then select IMM-542A and enter the quantity desired.

Temperature logs - In order to prevent financial losses and ensure the efficacy of vaccines, temperatures must be maintained within specific limits and should be checked twice daily; first thing in the morning and prior to leaving in the evening.
